Unable to receive calls on HTC EVO 4G - Callers getting "Not Reachable" message

Dear Experts, Have issues with receiving calls(says unreachable) and sms for last 10 days. Read this and other forums and tried lot to rectify but all are in vain. Not able to locate DDTM setting on Moto-G running kitkat, even after following instructions as directed here. It did not solve either changing "mobile network" to only CDMA(without evdo). Kindly suggest me a way out. Thanks in advance.
